Athletic Sport Physical Packet

The PIAA requires that all athletes receive a physical each year dated on or after May 1st. 
 
Physical packets can be obtained online or from the middle school and high school offices. There are also forms located outside of the athletic training room at the high school.  
 
Failure to turn in the following forms will result in being held from practice/participation until all paperwork has been received.
 
For your athlete's first sport of the school year, we will require the Complete Physical Packet. Each subsequent season will require the re-certification form. All forms can be downloaded by clicking on the blue title link.

Sports Physicals for the 2024-2025 school year will be held on Thursday August 1st from 5:00-7:00 in the HS Cafeteria.  Cost is $20.00.  Checks can be made out to LDHS.   You must use the sign up genius link below.

Once you have signed up for a time slot, please print and complete the Physical Packet below and bring it with you to your physical.
 
 
THIS ENTIRE PACKET MUST BE COMPLETED ONCE A YEAR TO PARTICIPATE IN SPORT AFTER MAY 1ST. 
  1. Complete PIAA CIPPE Physical Packet- It must be completed every school year after May 1st. Attach a list of medications in a sealed envelope.

 

WINTER  ATHLETES

Please complete this form if you have played a fall sport and return no earlier than 6 weeks to the start of the winter season.  If you did not play a fall sport you must complete a physical packet above and return it no earlier than 6 weeks to the start of the winter season.  

  1. PIAA Recertification form (Section 8) - Must be completed each additional season during the school year.  These forms need to be dated, no earlier than 6 weeks prior to the start of the second or third athletic season.  (Example: athlete played football in the fall and wants to play basketball and baseball.  Athletes will complete a re-certification form for the winter and spring seasons.)
