Sports Physicals

Physical Information
 
Sports physicals will be offered at the high school on Thursday, July 31st, 5PM-7PM. Cost per physical is $20.Payment must be received at time of appointment. Cash or check will be accepted. We are not equipped for electronic payment. Please follow the link below to reserve your spot. Paperwork must be completed PRIOR to your appointment. When scheduling more than one athlete per family, each athlete must be assigned a slot. We are not able to accomodate walk-ins.

ENTIRE PACKET MUST BE COMPLETED ONCE A YEAR TO PARTICIPATE IN SPORTS AFTER MAY 1ST. 
 
  1. Complete PIAA CIPPE Physical Packet- Pages 1-6 must be completed every school year after May 1st. Please attach a list of medications in a sealed envelope.

 

 

  1. PIAA Recertification form (Section 8) - Must be completed each additional season during the school year.  These forms need to be dated, no earlier than 6 weeks prior to the start of the second or third athletic season.  (Example: athlete played football in the fall and wants to play basketball and baseball.  Athletes will complete a re-certification form for the winter and spring seasons.) 
 
The PIAA requires that all athletes receive a physical each year dated on or after May 1st. 
 
Physical packets can be obtained online or from the middle school and high school offices. There are also forms located outside of the athletic training room at the high school.  
 
Failure to turn in the following forms will result in being held from practice/participation until all paperwork has been received.
 
For your athlete's first sport of the school year, we will require the Complete Physical Packet. Each subsequent season will require the re-certification form. All forms can be downloaded by clicking on the blue title link.
